MCCHD unveils new form for frontline health care workers to schedule COVID-19 vaccinations

MISSOULA — The Missoula City-County Health Department (MCCHD) has launched a new form to help identify and schedule appointments for frontline health care workers who have yet to receive COVID-19 vaccinations.

Montana’s Department of Health and Humans Services (DPHHS) allows medical providers who come into direct contact with patients are eligible to be vaccinated in Phase 1A.

The form is designed to help individuals determine their eligibility and get on the MCCHD’s vaccination list, according to a news release.

It is requested that health care workers who already have COVID-19 vaccination appointments scheduled elsewhere not fill out the form.

MCCHD notes that it's currently estimated that vaccines won't be available to the general public until late spring or early summer of 2021.
